OFA Application forms may now be created directly from your dog’s home page. It is frequently quicker to do so there.

If you have more than one dog, or something more complicated, it may be quicker to create the forms below.

Eye and Advanced Cardiac exam forms must be obtained during the visit to those professionals or you may start the process in OFA/Online if your vet participates.

OFA Certificates are sent for all normal/passing results. Non-passing results are sent when you do not already know the results from the veterinary visit. For example: abnormal cardiac and patella results may be added to your dog’s record, but we do not return reports for those.

DNA Databank

The CHIC DNA Repository collects and stores canine DNA samples along with corresponding genealogic and phenotypic information to facilitate future research and testing aimed at reducing the incidence of inherited disease in dogs.
